Requesting Medical Records
The Health Information Department processes all record requests for Harrison County Hospital and its Hospital-owned physician practices.
Harrison County Hospital retains medical records in accordance with Federal and Indiana laws and regulations. Information contained in the patient’s medical record is confidential. For this reason, all requests for medical records must include a signed authorization by the patient or patient’s representative.

How to request a copy of your medical records:
The Health Information department is located on the main floor of the hospital. Records can be picked up during the above listed hours of operation.
Photo identification will be required to assure the identity of the patient or patient representative prior to obtaining access to records.

Please note: Written requests can expect to take 10-14 business days to process. On occasion, extenuating circumstances applicable to certain requests may delay the turn-around time.
Is there a cost for obtaining copies of my medical record?
A fee is charged to cover the processing of requests.
Please Note: There is no fee when the copies are requested to be sent to your physician or another healthcare facility, as those requests are a part of your continuing care.
How do I obtain a copy of my imaging study?
The diagnostic imaging department is responsible for releasing all imaging studies to include:
